Equipment Assistance
This assistance measure is to provide financial assistance to enable equipment hire in the gaiing locality if the prosthesis or specialist equipment is not transportable and the qualifying conditions in the new locality prevent access to special equipment. Proof that the equipment is not transportable and evidence of placement on the new locality waiting list must be provided by the member at the time of application. To apply for assistance with equipment hire please contact your local DCO office and complete application form Form AC 836.
